Call 800-253-7852 and listen for the prompts.  DentalLink gives you step-by-step instructions to guide you to the Eligibility and Benefit information.  Please be sure to have your License Number and Tax ID Number ready as you will need to enter them into the system when prompted.  Press 3 after confirming patient information if you wish to request a fax document listing the benefits.
